The DOB and ID# fields HAVE TO MATCH what’s in the system – and it has to be the OLD 2016 ID#
(in the case where your ID# changed at the beginning of the year).
The 3rd field, the email address, needs to be a current email address that you can access now.
Because a verification code will be sent to that email address.
And you the member only have 10 min to access that code and to complete the verification
process – otherwise the system, for safety & security reasons, will time out.
